What NAD+ actually is

NAD+ (nicotinamide adenine dinucleotide) is a coenzyme present in every living cell. It participates in hundreds of enzymatic reactions and is central to energy metabolism — the pathways by which cells convert fuel into ATP. It is also the required co-substrate for two families of enzymes that have drawn a great deal of research attention: the sirtuins, and the PARP enzymes involved in DNA repair.

That much is settled biochemistry. What comes next is where the supplement industry tends to get ahead of the evidence.

About the “50% by age 50” figure

You will see it on almost every longevity page on the internet: NAD+ levels fall by around half between the ages of 40 and 60. This page used to say it too.

We went looking for the primary source and could not find one. The human measurements that exist are small, cross-sectional, and inconsistent between tissues — some studies report lower levels in older adults in skin or liver, while at least one large multi-cohort analysis of whole blood found no age-related decline at all. A 2022 review in Nutrients examining exactly this question was titled “Age-Dependent Decline of NAD+: Universal Truth or Confounded Consensus?”

So we have removed the number. What is fair to say is that NAD+ biology changes with age, that this is an active and genuinely interesting area of research, and that it is not a settled one. If that costs us a sale to someone who wanted a more confident story, we would rather lose it.

Two routes, and how they differ

NAD+ and NMN are related but not interchangeable. Here is the difference, stated plainly:

What the human trials have found

Two studies are worth knowing about in detail, including their limits.

  • NMN and insulin sensitivity. Yoshino and colleagues (Science, 2021) gave 250mg of NMN daily for ten weeks to 25 postmenopausal women with prediabetes. Muscle insulin sensitivity improved by roughly 25%. Body weight, blood pressure, blood lipids and liver insulin sensitivity did not change. The senior author said publicly that it was premature to make clinical recommendations from it.
  • A precursor and NAD+ in muscle. Elhassan and colleagues (Cell Reports, 2019) gave 1g daily of nicotinamide riboside, a related precursor, to twelve older men for three weeks. It raised NAD+ metabolites in skeletal muscle and lowered several circulating inflammatory markers. It did not improve mitochondrial function.
  • The underlying biochemistry is not in dispute: NAD+ is required for mitochondrial energy production and serves as the co-substrate for sirtuins and PARP enzymes (Verdin, Science, 2015; Imai & Guarente, Trends in Cell Biology, 2014).

What none of it shows yet: no trial has demonstrated that an oral NAD+ precursor produces a hard clinical outcome in healthy adults. Much of the striking work in this field — including the widely cited research on CD38 and NAD+ decline — is in mice, and mouse ageing biology does not transfer to humans automatically.

That this will fix fatigue, clear brain fog, or slow ageing. Those claims are everywhere in this category and none of them is established in healthy adults.
